I know not many people sit around and analyze this stuff, but I find it fascinating, and it would probably greatly benefit practitioners in our industry if they paid a little more attention to this sorta thing.

Most practitioners I talk to got into acupuncture to heal people, not to spend their weekends vetting acupuncture billing companies or staring at spreadsheets. I completely get that. But at the end of the day, your clinic is a business. And to keep your doors open and your stress levels down, you have to know where your revenue is actually coming from and where you want to spend your energy.

Recently, we decided to run the numbers here at Holbie. We pulled the data on over 145,000 active claims processed through our acupuncture billing services so far in 2026. We wanted to answer one very simple question: Which insurance companies are actually paying acupuncturists the best rates per visit?

The results were eye-opening. While the average reimbursement across the board hovers around $141 per visit, the top-tier payers are blowing that number out of the water.

The Leaderboard

Here is what the top 5 look like right now, ranked by average payment per visit:

  1. Cigna: $180 per visit. Cigna is currently the undisputed heavyweight champion for acupuncture. If you are seeing a Cigna patient, you are maximizing the value of that time slot and exceeding the national cash rate of $100 per visit.
  2. Veterans Administration (VA): $165 per visit. This is the most interesting data point on the list. Not only does the VA pay an incredibly high rate, but it also represents the highest overall volume. We’ve processed over $6 million in VA claims for our clients this year alone.
  3. United Healthcare (UHC): $149 per visit. A solid, incredibly reliable payer that consistently stays well above the national average.
  4. Meritan: $146 per visit. A surprisingly strong showing from a smaller network, proving that secondary players sometimes offer great fee schedules.
  5. Blue Cross Blue Shield (BCBS): $141 per visit. The "old reliable." They drive massive volume into clinics nationwide and provide a very steady, predictable revenue baseline.

What should you actually do with this?

If you are on the fence about insurance billing for acupuncturists (or are currently cash-only), you need to seriously consider making the leap. I talk to a lot of cash-based providers who avoid insurance because they assume the administrative headache isn't worth the payout.

But look at the numbers above. Are you consistently collecting $180 a session in cash without any pushback? For most clinics, pushing cash rates that high creates friction. Patients hesitate to book follow-ups, and your retention drops. Insurance patients, on the other hand, come back more often because their out-of-pocket cost is low, while your reimbursement remains high.

Yes, the red tape can be a nightmare. Credentialing takes time, and claim denials will drive you crazy if you don't have a dedicated acupuncture billing service in your corner or billing staff. But the math is clear: the revenue potential of accepting insurance far outweighs the cost of the paperwork.

Fine-Tuning Your Patient Base

If you’re already in the insurance game, this data is your roadmap. You should be strategically targeting patients who carry these top-tier insurance cards.

First, audit your credentialing. Are you participating with Cigna? If not, you are actively leaving the highest possible yield on the table. Second, lean into the VA Community Care Network. This is a massive, underserved demographic that pays better than almost any commercial plan.

Finally, update your marketing. If you know UHC and Cigna are your golden geese, make sure your website and Google Business profile clearly advertise that you are a participating provider for those specific plans.

At the end of the day, you can't control what insurance companies decide to pay. But you can control who you get credentialed with and who handles your claims. If you want to talk about how a specialized acupuncture billing company can help you align with these top payers, reach out. We love analyzing this stuff, so you don't have to.
